To help us take better care of you, Medicare will start sharing some of your
personal health information with us. This will help provide us with a more
complete picture of your health and be better able to coordinate your care.
If you want to stop Medicare from sharing this information, you should call

Your Medicare benefits aren’t changing. You still have the right to use or visit any
doctor or hospital that accepts Medicare, at any time. Your doctor may continue
to recommend that you see particular doctors for your specific health needs, but
it’s always your choice which doctors you use or hospital you visit.

Conveniently located a short distance from Ocean Medical Center, Jersey Coast Family Medicine is a state of the art facility designed to manage your family’s complete healthcare needs. We use cutting edge diagnostic equipment and an electronic medical record system to ensure the successful management of your health. Our electronic medical record system allows our courteous and caring staff to track, store and maintain your vital information so that you can stay up to date on important health items including: vaccines, screening colonoscopies, mammograms and other age appropriate screening tests. As an added benefit, most prescriptions can be sent electronically to the pharmacy of your choice.
Our patients have the ability to access their own health information via the Patient Portal. Via their web browser, patients are able to view and print their lab results, make appointments, e-mail the Doctor and pay their bill all in the comfort of their own home.
We offer secure messaging to both our physicians and staff. Please contact us thru your patient portal for any clinical questions, medication refills, insurance referral requests and/or billing questions.
Our physicians are available to our established patients 24 hours a day for all patient emergencies. Unsure if your emergency requires a trip to the ER? Please contact our office at 732-458-8000 and press 9 when prompted to reach our physician on call.
What to Bring to Your Appointment
• Your insurance card
• Your current medications (please bring your actual medication bottles)
• Recent x-rays, radiology reports or test results related to your condition
• Copy of your chart from your previous physician if available
New patients must complete their health history forms thru the patient portal prior to their appointment. If you have not received an email invitation from our office to register for your secure portal account please contact us.
Insurance and Payment Information
Jersey Coast Family Medicine LLC and its physicians are providers for Medicare and most major insurance plans. We bill insurance companies directly. You are responsible for charges not covered by insurance at the time of your visit. Also, your insurance company may require a co-payment at the time of your visit. When necessary, our staff will work with you to set up a payment plan. Please do not hesitate to call us with any questions you have with regard to your bill or which insurance plans we accept. Questions regarding your insurance coverage should be directed to your insurance company.
HIPPA and Patient Privacy
Here at Jersey Coast Family Medicine LLC we strive to maintain the highest levels of security with regard to your personal and health information. We follow government regulated HIPAA guidelines. As well, our electronic medical record is designed with security in mind. At your first visit, you will be asked to review our HIPAA policy and electronically sign the acknowledgment that you did so.
